DOT Physical

The Department of Transportation (DOT) physical is an in-person examination required for commercial drivers to ensure they meet health and safety standards. This physical includes a review of health history, blood pressure, vision, and hearing. If all requirements are met—covering vision, hearing, vitals, urinalysis, and physical exam—you’ll receive Form MCSA-5876 for Certification and be entered into the DOT National Registry on the same day. What do I bring to a DOT physical?

  • A complete list of all your prescription and over-the-counter medicines, including herbals, vitamins, supplements and essential oils.
  • Medical records from any specialists you see.
  • Visual and auditory aids like prescription glasses, contact lenses, and hearing aids that you use while driving. You’re allowed to wear these during vision and hearing assessments.
  • If you have diabetes, you’ll need a record (at least three months of your electronic glucose monitoring records/readings and your most recent Hgb A1C.
  • If you have sleep apnea, bring at least 90 days of data from your CPAP machine.
  • If you have heart issues, you’ll need a letter from your cardiologist saying you’re safe to drive a DOT vehicle. Bring the results of a recent stress test, ECHOcardiogram, or other tests within the past 1-2 years.
  • If you’ve had a stroke, brain tumor, seizure disorder, or bleeding in the brain, you’ll need a letter from your neurologist.
  • If you take prescribed medications, do not skip or stop taking your medication before the appointment, as this could temporarily affect test results and your health.

Waivers/Exemptions for DOT Disqualifying Medical Conditions

Only medical examiners listed in the National Registry can certify drivers during DOT physicals, but they cannot approve or issue waivers or exemptions for disqualifying medical conditions. Waivers and exemptions—such as those for vision, hearing, and seizure disorders—are granted exclusively by the Federal Motor Carrier Safety Administration (FMCSA). Drivers must meet specific FMCSA criteria before submitting an application and all required documentation directly to the FMCSA for review and consideration.
